Mine also same case... regsitered 5 years back... But gave executive exam in Dec 2009.... i contacted ICSI. They told there in one application available in website... Fill that application & send it along with a DD for Rs. 1500/-

there is a form called denova form which shall be downloaded from icsi website. fill it and send to students services at noida with DD for Rs.1500

yeh thr is a form named de nova registration.
