Multipurpose Empanelment Application Form for the year 2008-09 is expected to be hosted by 12th August 2008. 


            Everyone is advised to check their last year’s data and fill in all the blank columns and the relevant details.  This is for your own safety and benefit.


 


            New applicants should of course, fill in all the details of the form.
