You need to file Form TM-A along with certificate of incorporation, board resolution authorizing director to file application. Power of attorney is required if you want to file through agent/attorney. Logo is not mandatory but you may provide. If you want to claim 50% exemption of fees then obtain MSME UDYOG AADHAR.
