A candidate can apply for verification within 30 days from the date of declaration of results, necessarily in his own handwriting, giving specific details of the following along with the requisite fees:  Exam  Roll No  Paper(s)/subjects to be verified  Student Registration No  Address for communication

I have not mentioned adress of communication in my verification of marks application which I have submitted to the institute and I have made one more mistake I have given subect : revaluation of ca final exam , will any problem be created by institute please suggest me early

u can send it by speed post. no issues abt that. but dont send through courier. only normal or speed post.

there is no format. jus mentioned all the details possible. write a genral n a semi formal letter. stating that u r not satisfied wit ur marks n its not as per ur expectation. n mention d subject in which u need reval n pray 4 d best :)
