Go to library Mon.-Friday 10am to 5pm get the form from the office. Fill it u need IPCC or FINAL or Article Ship reg letter having a NCR Address. Fill the same address in form. If u dont have delhi ncr address then write a simple address change application in duplicate and get it stamped from ground floor. deposit it with the library application form. Wait for 7 days.    P.S. - Of course get ur draft of RS. 1000 ready before applying.
