Sir/ Madam,
I have applied for PAN card. I gave Driving license as proof of identity. But it shows my name with "alias" between 2 names. Similar is the case with other proofs like passport and voters card.
The PAN rules say my name on the form should exactly match with the identity proof. Since i cannot put alias on form, and also i dont have any proof without alias in my name, i am not quite sure what to do now.
Somebody please advice.

If u apply for PAN then proof attached with PAN Form should be same name which u hav Required.......
But In ur case, u can apply Without Alias if ur alias in form of ( ) between two name,
Otherwise, ur PAN will be same name as ur proof attached..

My name is "Vithal alias Vinil Sudesh Karmali" on all the proofs of identity. I cannot enter 'alias' on Pan form. So I entered my name as " Vithal Sudesh Karmali".
But now they are demanding a proof that has this name.( that is, without "alias" ).
Dear Vinil,
Do 1 thing get a counsellor certificate. It will act both as a identity proof and a address proof.
Tell the Counsellor to write the name as you are required.
