Need urgent help (online import export code - eiec )

I need to apply for import export code online. i have already created an ID through my PAN (individual's PAN) but i have a doubt and i am stucked badly.

i hold an individual PAN (i.e. not firm's PAN as i m a proprietor in my business firm), but i watched a tutorial uploaded by DGFT on youtube , the link is :

in this video it is said that in IEC MASTER i need to put "my name" in the field "ENTITY NAME"

My confusion is :

1. do i need to put my FIRM'S NAME or MY OWN NAME (that is on my PAN) as ENTITY NAME ???

2. I dont have any branch of my business firm... Is it necessory to provide branch details anyway ? is it mendatory ?

Mention name of the concern (not your name) under Entity Name
If there are no other branches, then either skip or mention nil (under branch) whichever the application accepts. Branch details to be filled up if there is any otherwise not applicable
