job work bill format

please suggest the Bill Format for Job Work....and should​ the gst will be divided between CGST and SGST for local job work also...

u have to generate delivery challan to t/f the goods to job worker

madam i do job work....should i need to maintain a separate book for job work bills....and how to mention tax on job work for local sales...
i.e, GST is 18% on job work....so CGST 9% and SGST 9% or something else...

u have to maintain separate books for each axtivities. for local sales mention cgst+sgst.
