Updating form 10-ee and Schedule FA if there is change in custodian account

Hi I Have for 401k and IRA accounts ( retirement accounts ) which are created when I was non-resdient. Now I am resident for many years and showing these accounts in FA schedule .  I need to change custidan now due to easy  use.  I will do direct rollover from old custodian account to new custodian account.   Question how to handle in form 10-ee which already filed for  old custodian accounts?. I heard we can't re-submit for 10-ee again. 

  Can I file new form 10-ee with  new  401k/IRA  account details. Will it not create issue since technically it  is new IRA account which will be created now ( when I am resident )  eventhough I am just doing rollover .  How to show this  rollver in FA schedule  

Any help on this will be greately appriciated. thanks

File a new Form 10EE for the new custodian account prior to filing your ITR. In Schedule FA, report both the old account (showing a zero closing balance after transfer) and the new account separately, maintaining rollover statements to prove continuity of the tax-deferred foreign retirement funds.
