It declaration form for fy 2012 - 2013

Hi All,

It is time for your employer, to ask you to submit Tax Declaration (Investments) for Financial Year 2012-13. I am attaching the IT Declaration form for the FY.  I have tried to include all components based on the latest union budget. 

The file is protected, for data uniqueness, but is not password protected.

Seniors, please correct me, if anything is not as intended.
