Help : IPCC Examination form

In "FORM A" (IPCC examination form)

There are two boxes at the end

1) Signature of member of ICAI / Gazetted officer

2) Name & membership no. of the member / Gazetted officer with office seal

 Is CA seal required along with name & membership number in 2) box ????

   who will sign  the examaniation form  if   a student converted  from pcc to ipcc ?

 i am student  of  pcc  and i have converted   into ipcc . My question  is that who will sign my  examination form ?

 prinicipal  or any other chartered accountant ?

Pls. give me the  reply  as soon as possibile
