Form 11 of epf act 1952

Lot of learned Charted Accounts have posted this message w.r.t. PF form No. 11

"

A declaration Form-11 is to be obtained from all employees joining the establishment.Whether their salary beloe 6500 or greater. Without maitain form No.11 epf dept treat to employee deemed under pf.and it will be penalised as per pf act."

Can any body provide me this law written in EPF act 1952. I need this very urgently

I am an Associate Professor in College and Fighting EPF case. I need this information immediately and if any body can provide any curt judgement on this will be great help and obligation to me
