Consent of nominee

Please filled up form INC-3 electronically then take Print out of it and take signature of nominee then scan it and attached scaned copy to INC-29. Form INC-3 is available for download on MCA site under attachment head

I confirmed from MCA - seva kendra they are saying there is no need to fill INC-3. only submit ID and Residential Proof.

Further In form INC-3, Point no. 1(a) saying SRN/CIN, both are unavailabe in OPC incorporation

have u submitted form then you got query for consent of nominee? R u filed form for OPC or Pvt Ltd. ? consent of nominee is mandatory attachment for OPC formation only. if you forming OPC then In that case I think just type Declaration part of form INC-3 on paper get it signed by nominee and attached it with ID & Address proof of nominee. 

I am in process of filled form INC-29 for OPC.

Is declaration on plain paper is sufficient for submission??

And please give me format of declaration also.
