DIN 4 must be filled & submitted to the MCA. As of now, online filing of DIN 4 is not available, I beleive & hence must be posted to MCA DIN Cell. A proof of ID & address supporting the new address & DIN 4 signed by director both must be attested by a CA or CS & must be sent.

Agree with Divya, Further adding to the above said queiry,

DIN 1 is for filing the application for obtaining the DIN

DIN 2 is for directors to inform the company about DIN,

DIN 3 Company should file the application to the ROC that they are the company directors.

DIN 4 is for changing not only for address but also any changes in the particulars of directors that should be informed to the ROC.

DIN cell address is available in MCA website that is noida.

Agree with Divya. Download DIN 4 from MCA website, fill it up, sign and attach necessary proof and then forward to MCA DIN Cell at Noida.

However my expereience is that no action is taken by MCA Din Cell, Noida in respect of physically filed DIN 4  unless strict follow ups are done. 

If you want to verify whether the changes have been accepted or not, you can download any form from the website which contains the particulars of Director. Just enter the DIN & give prefill. The changed information must appear. If not, then the changes are not accepted by them.
