Applying for form16a when only one payee

I m trying to request for Form 16A. But as number of deductee are less than three . An error is coming.

It is specifically mention on that page that if there are less than three deductee mention pan and respective amount for all.

In my case there is only one deductee and i have mention the same. But error is coming.

Kindly help.
