80DDB for specified disease

I want to take 80DDB deduction for the medical expenses of my dependent father who is suffering from chronic kidney disease. I have a prescripttion from the doctor. Do I need to upload this prescripttion/certificate or e-file any form such as 10-I to claim this deduction? 

Earlier there was an option for filing 10-I form, but now there is no such option.

No you need not to upload any documents, just take deduction under section 80ddb but you should maintain proper record.
