What forms required for registering for industrial training?

I have got an offer for industrial training in a company and have decided to go for it.

My questions except Form 104 which other form is required to be submitted to ICAI for registring as CA industrial Trainee??

Is it form 109 ? I am not clear about it, so please guide...

Hi there, U need to submit FORM 109 for TERMINATION OF ARTICLES (termination from where u r doing articleship right now) And Need to submit FORM 104 for REGISTRATION FOR AN INDUSTRIAL TRAINING (REGULATION 51)
