I received a mail from service tax department to submit few documents - in those document they have mentioned 1 Memorandum/Articles of Association/List of Directors. 2 Authorisation by the Board of Directors/Partners/Proprietor for the person filing the application. And business of the company is Management Consultant and Manpower

Now My company is Propreitorship firm

Please help me what should I do, is there any draft or format available - I don't know how to prepare

Articles, list of directors etc are needed in case of private limited companies
Since it is proprietorship concern, these are not applicable. Please also ensure that while filling up the application, the constitution is selected as proprietorship concern (not company)
Authorization is required in case of companies/firms etc. Since it is one man concern, proprietor himself can sign the application but if it is signed by some one other than the proprietor then authorization from proprietor would be necessary I think (not sure about this particular point)
