Sec. 44ad

The Return of income u/s 44AD with PPF interest, SB interest, devidend income and Agriculture income to be filed in which form.......................I am failing to file it in form 4S, The software takes  the form 4 as default to file this return. Can anyone clear my problem and state reason why I am failing in it.....................
