No, the format of such affidavits are not specified however, it must cover the required matter.

Thanks for the updation. However, from where do you get the information provided at point 19 of your write-up which is

19. "The Company shall within 30 days from receiving the order file the Certified copy of the orders to the ROC in form no. 21 alongwith the prescribed filing fees."

As Section 18 of Companies Act provides for 3 months time.

Secondly, with regard to your point no. 23 w.r.t publication in newspaper, what is source of this information.

I would really be grateful if you reply to my queries as stated above.

Thank you for the update. I have a doubt that i just gone through the procedure prescribed above and the earlier provision as well. Apart from change in CLC to RD and similar changes in complete process I could not find any other procedural change. Could you please let me know the major changes, if I have missed any??
