composition dealer under gst has to issue bill of supply.You can easily download its format from this site itself or you can also download the attached file.

very simple just like retail invoice under vat .
BILL OF SUPPLY
content BILL NO. DATE. GSTIN STATE CODE
CUSTOMER NAME. AND HIS GSTIN IF registered.
