I don’t think that there is any particular form for getting refund through ECS mode.
It’s the option that you should exercise at the time of filing your ITR. For that you should provide your Bank A/c No along with MICR Code and also, ECS refund is available only in selected cities.
If you have already filed your ITR without opting for ECS refund, your refund will only be dispatched by cheque/DD.
I don’t think there is any alternative such that you can change the method of refund, after you filed your ITR, other than submitting revised return.
