As far as last date is concerned, it's 01.04.2013 (Revised Cut Off Date, as actual date of 31.03.2013 is Sunday)

Since your profile says "None". I presumed that you are a fresher. But if you are a Graduate/CA-CPT/CWA Foundation passed candidate, then follow the instructions as given below by Bhargav.
