Regarding excess leave

HI :).. i completed my prescribrd period of articleship and submitted the articltship-completion form to the icai.. recently i got a letter from icai informing that there are excess leaves of 3 days which i hav to serve.. now kindly guide me that do i hav to re-fill and re-submit the forms(102-102-109..) for joining the articleship and then terminating it.. or do i simply write an application to the icai after completion of such 3days of articleship(attaching the letter sent by icai to me)?? kindly suggest.. thanks
