Non receipt of statement of marks

I have not receive my STATEMENT OF MARKS of PCC exams given by me in November, 2011.
  
Many time i have call to INSTITUTE'S helpline No. But no solution is provided by them.
 
Please provide me the Solution/Proceedure, what should i do to receive my STATEMENT OF MARKS.

Dear Nikhil,

 

You have to file a Application for duplicate mark sheet with an affidavit at rs. 100 or Rs.50 Stamp paper.Please confirm from the institute about the amount of Stamp paper.
