incorrect provisional id

i have been issued incorrect provisional id. the pan quoted in my provisional id is the one which i have surrendered to income tax department in 2012. i was issued new pan
and the same was provided to commercial tax department in 2012 . but the same has not been updated till date and i have been issued provisional id with old pan. pls suggest as time is running out and commercial tax department has refused to cooperate with us. please suggest
