Anyways, Write an application on simple paper to ur Assessing Officer with the correct particulars for reissue of refund order.....

Visit your jurisdictional Assessing officer with your necessary identity proof and collect the refund order by hand delivery.

A word of caution. Before taking the refund order, check the validity i.e. in case 3-months have  passed from the date of issue you will require to have the same to be re-validated.

Good Point by Rachit regarding "3 Months" cheque validity, usually people neglect the date part, and some think  it is available for 6 months like usual cheques
