my friend have the same query which was asked to cs institute the following reply was given
Dear student,
After scanning your documents such as attested qualification certificates, mark sheets etc., you are requested to send us the attested hard copies of the same along with payment receipt at our NOIDA office (C-37, Sector 62, Noida, U.P.- 201309) for our further action. On receiving attested documents, action will be taken for whole process.
For more details, you are advised to go through our web site www.icsi.edu / www.icsi.in for details.
Attested Hard Copies yet not received.
Study materials will also be sent after successful completion of registration.

it means no registration form can be downloaded
