what are the documents needed to claim refund of taxes on income for a salarized individual.. the company ahs deducted excess tax then what it should have deducted? 

obtain form 16 from employer, then file your ITR and claim refund of excess tax paid/deducted. 

Mr Sharma , The same has already been done. Now there is refund which is confirmed from income tax website also. the refund cheque got undelevered by saying that assessee has shifted...

get the pan data changed by fikling 49A, then write to your AO, coz without changing the address in pan , department will not listen to your request.
