I would like to know whether ONLY challan is required for payment of FBT or is any other form required. If at all any other form is required, Please tell me the Form Number and the category of assessees for whom the form is applicable.

for payment of FBT Tax by Challan no-283 & file to income tax department FBT return by ITR8
There is no need to have any other form along with challan. Some of the bank ask for PAN proof that either u can take search from income tax dept site or u can attach photo copy of PAN card
Its better pay the FBT online.Its only a 2m work. You can charge for it from your customer.
