For C form, you have to apply to the Sales-Tax assessing authority with whom you have registered as a dealer,
C forms can be issued only for items purchased for resale/processing/packing materialsetc.
there is no specific format in Kerala. Some nominal fees(stationary cost) is payable.
you have to keep a register showing C form issued- like C form no, to whom issued, purchase order no, value etc'
