Final registration

Respected Sir/Madam,

I am having some query regarding filling Final Registration form which are as follows :
1. Point No.9 Should I enter Date of Commencement of training in that & what proof should I attach with it.what does it applicable means?
2. Should I retain Acknowledgement receipt with me or Institute will return afterwards it with its stamp & should I fill only "THE SECRETARY ICAI" in Drawn on............. as there are not much blocks to fill full details.
