Final exam form queries

I have the following queries for my Final Exam Form:

1) Where do I get the Exemption Eligibility Certificate No. required to be provided in the CA Final Exam Form for Exemption in any paper ? I got my Mark Sheet recently, is it from there ?

2) Also since i have completed my articleship, can i get the Form signed by a Non-Practicing CA or does it have to be a Practicing CA..and that too does it have to be the one under whom i did my articleship ?

I request you guys to answer soon..cause I need to fill the Form as early as possible.
