Delivery Note, Please help me

Hi All,

Please Help on this

 

We are in to construction business, we have site in Tamil Nadu, we purchased some goods from bangalore for our Tamil nadu site, in purchase order we have metioned that delivery in our Tamil nadu site. In this case with delivery note is to be raised form 505 or 515 and who needs to be raised????
