The following link is for Faq's issued by ICAI on Verification of marksheet.

tushar the above notification is of 19 jan 2012 and that term there was no facility of making an online application The facility for making online application is being available made from this term only. In icaiexam.icai.org site they have said that we have have to take print of precribed format so i just wanted to be confirmed.

You have to write the format in ur own handwriting as mentioned on the top in dat format in brackets.

Jus write d given format and den fill it by urself and post it as mentioned der.

I took print out...filled it in my own handwriting (as they have mentioned) and then uploaded the file (96kb)..

@ ANIL i think dat uploading u did should be written by u even the basic format like that table asked but u get it confirmed from icai.

 Please download & print THIS DECLATARTION file on A4 size paper, Fillup the blank areas (in English if the Medium wasEnglish else in Hindi if the Medium of exam was Hindi) , Sign it, Scan as A4 (100DPI, upto 200KB). The signed scanned application which is clearly readable has to be uploaded.

U can call on this number and get it confirmed that whether u did is correct or not...just u may have to call it many times as no1 picks it bt in my case after 15 times they picked and answered it...ICAI Helpline(tollfree) -1800228009.
