You may use the following resolution:-

“RESOLVED THAT the signing authority of Mr. .........................................., Director be and is hereby removed from all the banks where he was authorized to sign the cheques and other documents on behalf of the Company.”

“RESOLVED FURTHER THAT the certified copy of above resolution be forwarded to all concerned banks for withdrawal of the signing authority as per this resolution.”
