I recently cleared my CA Final during the May2013 attempt.
I am from bangalore and i intend to apply for the membership number.
Can someone please tell me the procedure to apply the membership number.
I understand that Form2 needs to be filled, but which address should i be sending it to?

yes, you are required to file Form-2 along with the stated documents in it and also cheque/DD of Rs. 2000 (if only membership you opt for) and additional 2000 (if you go for COP also). Rest all correspondences after its submission would be mailed to you at your email ID or delivered at your home address which you stated in it.
In Bangalore, i think ICAI branch office:- No. 16/0, Millers Tank Bed Area, Bangalore- 560052 should be nearby your house... or any ICAI branch office which is near to your house or at the centre where you submits your Exam. Form and other correspondence, they can also accept the form for Membership.

Congratulations for clearing your Final examination. Hope you have completed your GMCS course. To apply for Membership, you need to download Form 2 and Form for “Issue of Identity Card” from ICAI website and accordingly fill all the relevant details applicable for you and send the filled up form and other documents to your Decentralized office. Please note the duly filled form and other attested copies be sent directly to Chennai Office, as the same is not accepted in Bangalore office.
The Fees is Rs. 4,500 (Rupees Four Thousand Five Hundred Only) in case of Membership only, including CABF. However, if you intend to apply for COP then you need to pay a total fee of Rs.6, 500 (Rupees Six Thousand Five Hundred Only).

Are you referring to Membership Certificate or Membership Number?
If it is Membership Certificate, then the chances of not receiving is quite possible, but tough to comprehend about the Membership Number, because Institute will definitely correspond with the members regarding it.
It is tough to provide a definitive answer based on hearsay, unless you share more facts regarding the case.
