Affidavit for signature problem

Hello,

I'm opening a Current A/c with Axis Bank but am facing problems due to my signature. Signature on my PAN Card and my Passport are different. I want to use the signature that I have on my Passport for my Bank Account. For this, the Bank is demanding an affidavit on Rs.100 stamp paper.

What is the format of this affidavit?

I, Hitesh (full name), son of (full name), Indian inhabitant, aged XX, residing at (full address), hereby declare and confirm the following:

I declare and confirm that the signature in my passport belongs to me. (sign)

I declare and confirm that the signature in my PAN card belongs to me. (sign)

I declare and confirm that the signature in the passport and PAN card are different.

I confirm that for the purpose of opening current account with Axis Bank, Branch, and that I shall be using the signature as per given in my passport. (sign)

I confirm that the above is to the best of my knowledge is true and genuine.

 

You need not write the exact draft as given above. But you can get an idea of what you need to declare in the affidavit.
