You have to give a letter in written form tothe concerned officer.

Dear Dheeraj,  You have file a amendement application i.e. in Form DVAT 07 alonwith a letter to change the records in central act . mentioning the date from which amendemnt will take, also it should be file within 30 days of change of address .

In dvat 07 you have to only mention the address to be changed and only basic information like name on the top and sign at bottom on all parts .

Also you have to attach a rent agreement (if rented) and electricity bill , also a NOC from surety dealer .

Then VAT inspector will visit at your new premises and then you have to got it corrected at your concerned ward
