I want purchase some goods from outside UP & also want to delivery at other than billing address then i want to know who will provide the form 38 either bill to or ship to. or there is any other requirement to do.

Itz purchaser who is reposible to provide the form . if the goods are brought in up state ..
At this stage the purchaser has the responsibilty or its the purchaser duty to issue the form 38 or form C against the bill.
Mr. Hashim, you'll provide form 38 for importing of goods in U.P.
