Form-XXXVIII is a transit form which must accompany the consigment brought into state of U.P. from out side the state. I think department is issuing these forms online now. You hv to get in touch with department or your counsil to get fimilar with formalties in this regard.
Form C is a form which entitltes the purchase of goods at concession rate of 2% from outside U.P. provided goods are declared and allowed to be purchased by the Assessing Authority in CST registration certificate.
Department issue blank forms to registered dealer under CST act and you need to keep the utilisation records in prescribed format
