you want registration in name of proprietorship FIRM, so you have to produce any document where the address of proprietorship FIRM is mentioned, 

Trade Licence is a better option. 

it is very easy just create login id in aecs website and fill the form ST 1 and submit it online, and take printout and take

PAN CARD (self attested with seal)

ADDRESS PROOF (Self attested with seal) (telephone bill, rent agreement anything)

CERTIFICATE OF REGISTRATION (incorporation shops act, distributors act)

copies of above documents and go to service tax office, and submit they will just approve ur online application and by afternoon you can login in aecs website and take printout ;) very easy thing
