There is no prescribed format i.e

Name, Address, Date, Service-Tax No. if having, Gross Amt. Service Tax Amt. Ed. cess, S & H Ed. Cess, some naration which period, Ath. Sign., and some instructions which are mention on the invoice. It is not mandatory.
