Radhika, Basic requirement is to clear 12th Standard after that you can appear for CPT exams. (provisional registration can be done after clearing 10th).
CPT is entrance exam for entering in CA Course. So, no other exams are required to give. Please download admission form from the following link and fill after reading instructions.
