Provisional registration

i am bcom final yr student.i hav written my final yr exams n awaiting my results.i want to apply for provisional registration for ipcc both groups...

for demand draft “T he Secretary, The
Institute of Chartered Accountants of India” payable at ....what should be the location Mumbai/ Chennai/ Kolkata/ Kanpur/ New Delhi...i m from hyderabad and what should be the amount of dd and does it include any cost for form which i downloaded frm icai website???
