Plz reply - can i use this form now??

Hello, i purchased the articleship training registration form in march this year i.e. 2013 but didnt join any firm and waited for group 2 exams but now i wantto join a ca firm for training.So, my question is, can i use that form now for registration OR that form is as per exam attempt and i have to purchase another one? please guide me. thank u.
