Please help on the transfer to new final course

I m a final year old course student. I hv cleared my 1st group but not the second one.Please anybody tell me what should i do to transfer myself to new course. what is the procedure for the same. Also i wanna knw whether i cud get the revised study material and where cud i get the hand delivery of the same.What is the prescribed Form i have to fill in and submitt to the Institute. Please help me as it is very urgent for me.
