if a photographer served his photography service and the party ask for gst number for making of payment then is he opt for gst registration ?

Then register recipient is liable to pay tax under RCM
recently i have worked with bank... now he have to make payment.....at that time he ask me for gst registration number and i told them that my turnover is lessthan 3 lakh.... then he said for submit of non gst registration certificate
where should i get it ?
Provide self declared certificate attested by notary
sir do me another help... is there any format available for self declared certificate ?
