Can u please give me a format of the notice to be given to the Director who has not attended 3 consecutive board meetings.

The director will vacate his office of director by operation of law u/s 283(1)(g). You have to place the matter before the Board for information and file Form 32 with ROC. You should also intimate the director concerned accordingly.
