It is investment declaration which every employee has to submit to his employer for his tax planning..

there is no such investment form from department 

so jackin prepare a form in excel and word sheet with the some coloumns where you can mention some sections under which a individual can get deductions from GTI and section like.

80 C (you may make further sub coloums for tution fees, LIC, PPF, PF etc)

80 D 

80 G

80 GG

80 U

some other applicable sections.

Please find sample of last F.Y. as attachment, same i need for current F.Y.
